Intake Orientation and Assesment

Each parent must complete a separate intake assessment before services begin. During the interview, we will discuss your reasons for needing supervised visitation and any family issues that may impact parent-child contact or the safety of all participants. We will also inquire about any ongoing medical conditions affecting health and safety.

 

Before services commence, the supervisor will explain program policies and procedures, confidentiality limits, and expectations to each parent. Both parents must sign a service agreement.

 

To maintain neutrality, the supervisor prioritizes the needs of the children. Issues between parents remain between them.

 

Please bring the following items to your intake meeting:

  • The Intake Packet

  • Photo ID for yourself and anyone authorized to pick up/drop off children

  • Court-ordered documents regarding supervised visitation (email large files to angeladoble@yahoo.com)

  • Restraining order, no-contact order, or stalking order (if applicable)

  • Recent individual photos of the children participating.

  • Any additional information to facilitate safe and effective supervised visitation.

 

It's helpful to write down questions beforehand.

 

Parents are responsible for informing us of any court dates or modifications regarding supervised visitation. Copies of modification orders in effect by the court must be provided.
